  • 3 years or more of obesity
  • BMI of 40+
  • BMI of 35+ with comorbidity
  • Between the age of 18 and 60
  • Failed at other weight loss options
  • Have an understanding of possible risks and side effects
  • Open to making significant changes in your lifestyle
  • No history of alcohol or drug abuse
